  Application for Extended Leave

This form must be completed for family holidays and travel outside of school holiday period. Once complete, please return the signed form to the school office.

  Anaphylaxis Action Plan

This action plan was designed for students with Anaphylaxis. The office maintains paper copies of these forms, though, students with Anaphylaxis should keep a copy of this action plan as well.

  Asthma Action Plan

This action plan was designed for students with asthma. The office maintains paper copies of these forms, though, students with asthma should keep a copy of this action plan as well.

  Diabetes Action Plan

This action plan was designed for students with diabetes. The office maintains paper copies of these forms, though, students with diabetes should keep a copy of this action plan as well.

  Medical Administration Form

This form must be completed to supply permission for the office to administer medication to students. The form must be completed and returned to the office.

  Out of School Achievements

St Madeleine’s Primary encourages the successes of children by sharing their achievements via our school newsletter. If your child has gained a special ‘out of school’ achievement and you would like to share it with the school community please complete and submit this form.

  School Term Calendar

This calendar outlines the school terms as well as school holidays throughout the year. Staff development days are not included in this calendar and will be advised by the school throughout the year.

  Student Acceptable Use of Technology Policy

The use of ICT within the school should be responsible, legal, appropriate, and for educational purposes. Usage must follow the guidelines outlined in this policy. Both students and parents/caregivers must read and sign the student agreement and parents /caregivers’ policy. It must then be returned to the relevant Classroom Teacher as soon as possible, during Term 1 of each year.

Child Protection for Parent Volunteers

Please note that all volunteer workers in schools are required to undergo child protection training. Visit the Parramatta Catholic Education Office website 'Building Child Safe Communities' for more information.
